By signing my name above, I am choosing to participate in services offered by Tina Ojeda-Willis of Reiki Warrior Healing. These sessions may include Reiki, energy work, sound therapy, somatic movement, and other holistic healing practices intended to support relaxation, emotional release, and overall well-being.

Nature of Services

I understand that these services are holistic and non-medical in nature. They are not a substitute for medical, psychological, or psychiatric care. Tina Ojeda-Willis is not a licensed medical provider or psychotherapist and does not diagnose, treat, or cure medical or mental health conditions.

I take full responsibility for consulting a licensed healthcare provider regarding any medical or psychological concerns. No specific results or outcomes are guaranteed.

Assumption of Risk

I understand that sessions may involve physical, emotional, or energetic responses, including sensations in the body or emotional release. I voluntarily choose to participate and accept full responsibility for my experience.

Medical & Safety Awareness

I agree to inform the practitioner of any relevant conditions that may impact my participation, including but not limited to:

• Pacemakers or implanted medical devices

• Pregnancy

• Injuries or physical limitations

• Mental health conditions

• Sensitivities or allergies (including to essential oils)​

I understand that sound healing, energy work, and somatic practices may affect each individual differently.

Ceremonial Cacao (Optional)

I understand that ceremonial cacao may be offered in some sessions and is completely optional.Cacao is generally considered safe but may have mild stimulating effects. I take personal responsibility for choosing whether to participate and will communicate any relevant health conditions, sensitivities, or medications.Initial here if you choose to participate.

Confidentiality

All personal information shared during sessions will be kept confidential, except where disclosure is required by law (such as risk of harm to self or others). Any sharing of client experiences for educational or professional purposes will remain anonymous unless explicit consent is given.

Release of Liability

To the fullest extent permitted under California law, I release and hold harmless Tina Ojeda-Willis, Reiki Warrior Healing, and any associated staff, contractors, or facilities from any and all liability, claims, or damages arising from my participation in these services.
